(Adnkronos) - In 10 anni sono aumentate le concentrazioni di fentanyl, metanfetamina e cocaina nei campioni di urine, mentre l'eroina è diminuita. È il dato che emerge da uno studio pubblicato su 'Jama Network Open' che mostra come sia cambiata radicalmente nell'ultimo decennio l'esposizione a queste sostanze, specchio anche dell'evoluzione del mercato della droga. La fotografia è scattata negli Stati Uniti, un contesto che spesso anticipa tendenze in grado di diffondersi a livello globale, interessando anche l'Europa (come nel caso del fentanyl).Â
Lo studio si basa sui dati relativi a quasi 922mila campioni di urina unici raccolti tra il 2013 e il 2023. Il cambiamento osservato nel panorama delle sostanze utilizzate, evidenziano gli autori, rende necessarie iniziative di sorveglianza rafforzate e rende urgente accelerare gli sforzi per trattare le persone esposte a fentanyl e stimolanti illegali. In questi anni gli Usa stanno vivendo una crisi prolungata causata da overdose principalmente associate all'esposizione a fentanyl prodotto illecitamente, metanfetamina e cocaina, ricordano gli autori della ricerca. Il rischio di overdose e le risposte al trattamento possono essere direttamente influenzate dalle concentrazioni assolute dell'esposizione alle sostanze e dalla prevalenza d'uso. Â
Per questo gli esperti della Johns Hopkins University School of Medicine e di Millennium Health hanno deciso di avviare l'analisi concentrando la loro attenzione sui risultati dei test urinari per la ricerca di sostanze da campioni raccolti tra l'1 gennaio 2013 e il 22 agosto 2023, in 49 stati e nel Distretto di Columbia. I campioni sono stati ottenuti da pazienti di età dai 18 anni in su, che si sono presentati nelle cliniche per il trattamento di disturbi da uso di sostanze. Il test è stato disposto dai medici in base.Â
L'analisi è stata condotta per più sostanze o metaboliti - fentanyl, eroina (6-monoacetilmorfina), cocaina (benzoilecgonina) e metanfetamina - utilizzando la cromatografia liquida con spettrometria di massa in tandem. I campioni appartenevano per il 59,6% a maschi, età media 34 anni. La concentrazione aggiustata di fentanyl nei campioni di urina era di 38,23 nanogrammi per milligrammo (ng/mg) di creatinina nel 2023, rispetto a 4,61 nel 2013. La concentrazione di metanfetamina era di 3.461,59 ng/mg di creatinina nel 2023 contro 665,27 del 2013. La concentrazione di cocaina è stata di 1.122,23 ng/mg di creatinina nel 2023 e di 559,71 nel 2013. Tutti valori in crescita (soprattutto il fentanyl). La concentrazione aggiustata di eroina è risultata invece in calo: è stata di 58,36 ng/mg di creatinina nel 2023, rispetto a 146,59 del 2013. Â
